Evidence Levels
High: Multiple attributable primary materials or official records support the core claim.
Medium: Substantial documentary support exists, but one or more factual elements remain incomplete or contested.
Preliminary: Early materials indicate relevance, but the record is not yet sufficient for a full published case entry.
Inclusion Standard
Cases are included where the underlying event is verifiable, materially relevant to operators or resource holders, and significant for understanding systemic governance or registry risk.
Loss Estimation
Estimated economic exposure is conservative and may include operational disruption, legal response costs, delayed transfer value, transaction uncertainty, and governance-related friction.
Anonymous Submissions
Anonymous and confidential submissions are permitted. Identifying information may be withheld where publication would create additional operator risk, provided the factual record can still be evaluated.
Corrections and Updates
Entries may be revised as new primary materials emerge. Substantive corrections, changed status markers, and updated exposure estimates are reflected in the archive record.
